Welcome to New Patients at EHMC

Welcome to Elliott Hall Medical Centre!

To register as a patient at EHMC – please click here to read more information and download the necessary forms. Please note that it will take several weeks for your paper medical records to arrive at EHMC.

Once you have been registered – you will receive a text from reception confirming that your forms have been processed. Please be aware that this can take up to 5 working days.


To arrange an appointment with a doctor – please complete an PATCHS. This will be triaged by the practice and you will be contacted within 48 hours normally via phone or text message.

If you are on regular medications

If you are on regular medications – your records will be reviewed by a doctor or practice pharmacist who will contact you if need a review. For patients on regular medications, in most cases we perform medication reviews every 6 months. This may be more or less frequently depending on the type of medication. You will receive a text message or a note on your prescription to remind you to make an appointment for your medication review, please ensure that you make these appointments, as without regular reviews we may need to reduce the amount of medication we are able to safely prescribe.

Requesting Prescriptions

Please register for online access using the NHS App. This will allow you to request your medications online and we aim to process requests in 2 working days.
