How to Register
To register with the practice you will need to complete a registration form, which you can do online
We are using a new online service that makes it easy to register with Elliott Hall Medical Centre
Just fill in this quick online form to start the process.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one via our reception
We will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case you will need to choose another GP in your local area.
Please note that your medical records may take several weeks to arrive at the Practice. Further information concerning your registration with us, is contained within the document shown below “What happens after you have completed your Registration Pack”

The National Care Record Service (NCRS)
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
