Attendance
Allowance is for people over the
age of 65 who need a lot of looking after. They must need care because of
illness or disability. Attendance Allowance takes account of how much care the
person needs. It makes no difference whether the person is actually getting the
care. A person may be eligible even if they live alone. Attendance Allowance
does not depend on a person's finances. if someone
lives in their own home, Attendance Allowance does not cause any other
benefit to be reduced.
When you have received and completed your
part of the form, if you would like one of the doctors at Elliott Hall to fill
out the part required to be filled out by a health care professional. Hand it
into the surgery, you can either include all the information and envelopes etc,
in which case we will send it off for you, or allow approximately one week and
collect the completed form from reception. There is no charge for this
facility.

Disability
Living Allowance is for people

An employer pays statutory sick pay (SSP) to staff who are off sick. Most working people are covered by this
scheme but those who are not, may qualify for Incapacity Benefit or Income
Support paid by the Benefits Agency.

Disabled
road users come into two
categories, those who require a European blue badge and those who require a
taxi-card. You should apply to Social Services for either of these forms. First
you should complete the patient's section, checking what photographs and fees
are required. There is part of the form which must be completed by a Health
Professional, the Doctors at the practice will be happy to do this for you. You
can either bring the form, photographs and payment complete with forwarding
envelope, we will then fill out the Doctors section and forward it to social
services, or leave the form with us and collect it, allowing approximately one
week.
