 CLINICS AND SERVICES

Asthma
 
The clinical team in general manage patients with asthma.
Patients are invited to attend for periodic review if they are on regular treatment.

 

Chiropody

Jemma Klein is in attendance at EHMC on Mondays pm and Thursdays pm.
Appointments (Private only) on 8922 0426 or 8203 3078.

 Community Midwife.

The midwife is based in the Community Midwives Department in Northwick Park Hospital.
After confirming a pregnancy the Doctor will refer the patient to the midwife for an hour long appointment, which
is arranged by the midwife.
The ante-natal clinic on alternative Fridays 1.30 - 3.30 pm.

Women who who wish to give birth at Northwick Park Hospital will be  booked
at the Medical Centre by the midwife.
Shared ante-natal care will be carried out between the community midwife,
GP and NPH.

Parent education classes are held at Cecil Park Clinic and NPH. The contact
number at NPH is 8869 2871/2

 Coronary Heart Disease Management

ECGs and Blood Tests are carried out by Sister Allison Willis on a Friday.

 

Counselling In General Practice

 We are awaiting the appointment of a new Counsellor at the  Psychology Department at Northwick Park Hospital before we can resume our inhouse clinic.
 

Note that Government figures indicate that up to 30% of the problems that patients consult
their GPs about have a psychological component. Studies suggest that counselling is popular
among patients and the majority of patients benefit from a psychological approach.

Patients who are experiencing emotional problems may be referred by their GP to the Practice
Counsellor, who can offer an assessment and brief psychological treatment, if appropriate.

Issues that patients bring may include depression, bereavement, anxiety, stress, relationship
problems and difficulties adjusting to recent crises and life events. The first one or two
appointments will be for assessment purposes.  It presents an opportunity to talk about the
problems is some detail with the aim of deciding how best to offer help . Questions that may be
asked include the nature of the problems, the period for which they have existed and past
treatments. The Counsellor may ask about different areas of the patient's life in order to build up
a picture of the person, not just the problems. Depending on the problems, support and counselling
will be offered or a more active treatment pursued. Sometimes patients may need to be referred
to another agency specialising in a particular difficulty.

Cryotherapy

There are fortnightly sessions carried out by Sister Angela Bradley.

Diabetic Clinic

The Diabetic Clinic is at the moment held on a Friday morning. It is organised and taken by either Dr Jenner or
Dr Kelshiker and Sister Bradley.. The aim of the clinic is to monitor and to treat all our diabetic patients.
You will be contacted by Sister Bradley , who will give you a date and time for your appointment. She will also
send you a urine test form and if necessary a blood form. The results of these tests are required at your appointment.
Please arrange to have your blood test done  at least ten days before the date of your  diabetic clinic appointment.

 Dietician

The dietician is in the surgery on alternate Thursday afternoons. Patients are referred to the dietician by the doctor and appointments can be arranged subsequently at the reception desk.

The dietician is based at Northwick Park Hospital , Nutrition & Dietetics Department, but runs a clinic once every two weeks at EHMC.  Patients are seen by referral from a Doctor or Nurse Practitioner .
Some of the conditions that benefit from this service are:

     Diabetes, Coronary Heart Disease, Obesity, Irritable Bowel Syndrome,
     Underweight and unexplained Weight Loss.
 

District Nurse and Community Nurse.

Based at Cecil Park Clinic the District nursing team support the practice by offering care for patients in their own
homes.

Messages are collected at 8.30am, 10.30am, 2.30pm and 4.00pm.
Messages left after 4.30pm will be dealt with the following day.
  

Health Visitor

The health visitor is based at EHMC and is responsible for development checks on babies and children. She also runs a drop-in clinic on a Thursday between 12 and 1pm at EHMC.
As the Harrow Primary Care Trust have decided to change the structure of child development checks, all babies
will be offered a check with the Doctor and the Health Visitor at 6-8 weeks old.
Routine checks with the Doctor will then cease but there will be a Health Visitor contact at 8 months and 2 years old.
This will give the Health Visitor  more time to spend with needy families.

Any parents with concerns can continue to make contact with the Health Visitor at any time.

Immunisation Clinic - Children

                     

This clinic is now run every Friday afternoon from 1.30 pm,  by appointment only.

There is a lot of information
available at the surgery to help you make the right decision for your child, based on the facts.

The Department of Health have a  Haemophilus Influenza type B (Hib) Vaccination catch-up campaign.

 This is in response to a recent rise in cases of diseases caused by Hib infection.

Children requiring immunisation between the ages of six months and four years will be

called for immunisation in the near future.

Macmillan Nurse

Macmillan nurses are on hand to provide support for cancer patients.
The Macmillan registered nurses are specially trained staff, who have a wide range of education and experience
in cancer nursing, district nursing and palliative care. They are in contact with the local GPs and work as part
pf a multi-disciplinary team of health and social professionals, co-ordinating  care and providing continuity between
hospital, hospice and home.

The level of involvement of the Macmillan nurses can vary from telephone advice to a single meeting or continued support over a longer period, depending on individual needs.

Mental Health Care

In House Mental Health Care

 The service, provided by attached Primary Care Mental Health Staff, is for anyone experiencing mild to moderate mental health problems, such as feelings of depression, anxiety and panic, which affects a majority of people at some point during our lives.

If you are finding it difficult to manage relationships, work, family, or any number of issues you feel you need help with, we can provide you with an opportunity to talk and be listened to.

 

What type of service can you expect?

There are many ways we can help ourselves. As Primary Care Mental Health Workers we will show you how to use some useful techniques and tools to help address any difficulties you are experiencing, such as step by step guidance in working through Cognitive Behaviour Therapy Self Help Manuals with you, and counselling using a person centred approach.

 

What are Cognitive Behaviour Therapy Self Help Manuals?

 These are manuals focused on trying to challenge and change your thinking through several different tools. It is an active way of dealing with any issues you are experiencing, We can help you with each stage of the manual, but some homework will need to be completed for it to be an effective treatment for yourself.

A majority of the self help manuals can be downloaded from "Contents" on the EHMC Website.

 

What is Person Centred Counselling?

Person Centred counselling allows you to talk about any issues you feel are bothering you at the time. Person centred is not a directional type of counselling, like CBT Self Help Manuals. CBT is quite focussed on particular issues, whereas Person Centred is focussed on you and how you feel and reflecting on parts of your life.

If you are looking for a more directive approach CBT Self Help

 

Manuals would probably be more beneficial, but if you are looking for a non directive approach, Person Centred is probably better suited to you.

During your assessment with a Graduate Primary Care Mental Health Worker, an approach using CBT or person centred will be discussed, so don't worry about which approach maybe best suited to you prior to the first session.

How do I access the service?
To access the service, you need your GP to refer you. Once you have been
 referred a Graduate Mental Health Worker will contact you to arrange an appointment.

You will be offered from 2 to 8 sessions, each lasting approximately 50 minutes.

An assessment will be done in the first and last session to assess the level of your mental health problem. A one-month follow up following the last session will be offered to assess your progress and to see if further assistance is needed, such as referring you on to relevant voluntary agencies.

 

As it is a new service, some of the issues you experience may not be able to be dealt with via a Graduate Primary Care Mental Health Worker and if this is the case you will be referred back to the GP.

All the sessions will take place at Elliott Hall Medical Centre.

Confidentiality
The sessions are confidential to the Graduate Primary Care Mental Health Service, to your GP and a Clinical Supervisor. The Graduate Mental Health Worker works with your GP as part of the primary care team and may speak to your GP from time to time about your progress.

 

Graduate workers also undergo clinical supervision, where clients are discussed with a qualified therapist to assist in developing our role and to help meet your needs.

 

No information will be passed on to anyone else without your consent, unless the Graduate Mental Health Worker is of the opinion that you are at risk of harming yourself or others. Brief notes will be taken and recorded in the practice system.

Evaluation
As this is a new service it needs to be evaluated and this will be done via a brief questionnaire at the end of contact with the service. The information will be treated in strict confidence. No names will be used on any of the questionnaires. It is purely to help with our assessment of your problems and assists in evaluating how the service can improve.

This participation is voluntary. If you do not wish to complete the questionnaires, it will not affect your use of the services.

 Most tests can be carried out inhouse, but you will be asked to make an appointment at the Wealdstone Centre if you need a Glucose Tolerance Test.
